This pre-screen form is used to determine basic eligibility for independent shared housing for male veterans age 62 and older.
The information provided is used to assess independence, housing compatibility, and readiness for placement.
Submission of this form does not guarantee housing and does not constitute an application or lease agreement. All placements are subject to availability and program review.
Case managers may submit this form on behalf of eligible veterans.

Program Eligibility & Placement Review
This housing program is designed for veterans who can live independently in a shared environment.
Placement may not be approved if a veteran:
- Requires medical, nursing, or assisted living care
- Is unable to live independently in shared housing
- Presents a safety risk to themselves or others
- Has a history of repeated violent offenses
- Has active legal restrictions impacting housing eligibility
- Has been dishonorably discharged
- Does not meet overall program participation requirements
Eligibility is reviewed on a case-by-case basis.
